• Review of previous injuries, surgeries, and current medications
• Completion of required school or sports physical forms
What to Bring
Please bring the following to your appointment:
• School, sports, camp, or activity physical form, if applicable
• Current medication list
• Glasses or contact lenses, if worn
• Information about significant medical conditions, previous injuries, or surgeries
• Relevant immunization records, if required by your school or program
• Parent or legal guardian for minors, when required
Important Information
School and sports physicals are focused examinations and do not replace a comprehensive annual wellness visit.
If a health concern is identified during the examination, additional evaluation, testing, treatment, or specialist clearance may be recommended before participation. Additional services are not included in the $55 physical fee.
