Screening tests for high blood pressure, diabetes, high cholesterol, thyroid disorders, anemia, vitamin deficiencies, sleep apnea.
Chronic Routine Health Care
Limited to three stable chronic disease conditions **
Acute Care Evaluation
Common cold, flu, COVID, sore throat, strep throat, conjunctivitis, sinus infection, ear infection, seasonal allergies, urinary tract infections, post-exposure sexually transmitted disease care, sprains, strains, cellulitis, sun burn, scabies, hives, ring worm, contact dermatitis (poison ivy, poison sumac) constipation. ***
Behavioral Health
Anxiety, depression, postpartum depression, relationship counseling, trauma and loss, eating disorders, screening for above conditions
Chronic Skin Conditions
Rosacea, acne, folliculitis, cold sores, eczema, cellulitis, abscesses, sunburns, rashes
Contraception Care
Oral contraception, patch, Nuva ring, pregnancy testing, preconception counseling and preparation for pregnancy
Weight Management
Nutrition counseling, exercise recommendation, pharmacotherapy, goal development and benchmarking success
Definitions
Hypertension =high blood pressure
Hyperlipidemia = high cholesterol
Hypertriglyceridemia = high triglycerides
Hypothyroidism = low functioning thyroid
Chronic Kidney Disease [CKD] will be discussed further as applicable
* Well child visits include exam only-we do not have the resources to provide vaccinations at this time
** Stable Chronic Disease conditions may include diabetes, hypertension, hyperlipidemia, hypertriglyceridemia, asthma, chronic obstructive sleep apnea, obesity, osteoarthritis, vitamin deficiency, hypothyroidism, chronic kidney disease [up to stage III without elevated BUN or creatinine]
